Provide written instructions to agency staff for the care of ill or injured youth. 7. Initiate all referrals to health care providers and assess the results of therapeutic treatment. 8. Responsible for scheduling of all health care needed by youth. Will ensure that providers have appropriate billing information. 9. Assists in the control of communicable disease by the prevention, early detection and referral as appropriate. 10. Acts as a liaison between parent, school and the community in health matters as appropriate. 11. Confer with Food Service Manager concerning individual diets prescribed and nutritional concerns, consults with the dietician as needed. 12. Obtain records from previous providers and maintains accurate and complete health care records for all youth, including the electronic health record, according to regulations and the New York State Education Law as appropriate. 13. Assesses youth and reports adverse reactions to medication or treatment to physicians or other prescribing health care providers. 14. Acts as the liaison to Social Workers concerning health needs and concerns of youth. 15. Coordinates consulting psychiatric services. 16. Work collaboratively with the treatment team to ensure that youth’s health care needs are met within the context of the multi-disciplinary treatment plan. 17. Develops individual health care plans with specific goals, objectives, and interventions for youth with special health needs or chronic health problems. 18. Ensures that discharge and/or transfer health summaries are completed for youth leaving the agency 19. Provides in-service education for staff when indicated, and provides any needed training concerning health related activities to unlicensed staff. 20. On-call for health-related concerns and provides triage/recommendations for staff. 21. Maintains current First Aid/CPR and Therapeutic Crisis Intervention certification as provided by Agency. 22. Provides Medication Administration Certification training to unlicensed personnel who assist with the administration of medications. 23. Performs duties in a manner consistent with professional standards and the NYS Nurse Practice Act and other local statutes or regulations applicable to nurse practice, inclusive of school nurse standards. 24. Reports suspected Child Abuse and Maltreatment as mandated by law and agency policy. 25. Attends all mandatory in-service training and regularly attends supervision meetings. 26. Maintains professional development through self-directed professional growth activities designed to increase professional knowledge. 27. All other duties as assigned by immediate supervisor. SKILL, QUALIFICATIONS, KNOWLEDGE, ABILITY &amp; PHYSICAL REQUIREMENTS: Minimum Education and/or Professional Qualifications/Skills • Current licensure in NYS as a Registered Professional Nurse. • Previous experience in pediatric nursing or a childcare setting is preferred.
